We’re glad you’re here! Your membership gives you direct access to primary care without per-visit office fees, copays, or surprise billing.

Before scheduling your first new patient visit, we like to review your prior records and recent bloodwork whenever possible. This helps us make your first visit more useful, focused, and personalized.

Step 1: Learn how to use your patient portal

Your Cerbo patient portal is the best place to message us, upload records, complete forms, and review your care plan.

Step 2: Send us your medical records

The fastest option is to download your records from your prior clinic or health system and upload them directly into your Cerbo portal.

If you prefer, you can complete the Authorization to Release Information form in your portal, and we’ll request records from your previous clinic. Once we receive them, we’ll reach out about scheduling your new patient visit.

Step 3: Make sure we have recent labwork

If you’ve had bloodwork done in the last 6–12 months, we can usually start with those results. Please upload them to your portal or make sure they are included with your records.

If you haven’t had recent labs, we can order a baseline lab panel through LabCorp for $25. This includes:

CBC, CMP, lipid panel, TSH, and A1c.

What happens next?

Once we have your records and/or labwork, our team will help you schedule your new patient visit. This allows your doctor to review your history ahead of time and make the visit as helpful as possible.
