Our model allows us to spend the time you need, handle most medical issues right in the office or online, perform minor procedures, and reduce unnecessary specialist referrals. Our Story

I’m Nicholas Jones, MD, a board-certified physician who left traditional insurance-based primary care after seeing firsthand how little control doctors had over patient care. Frustrated by the shift from patient-focused medicine to insurance-driven metrics, I founded Clear Health Direct Primary Care on November 6, 2023, starting with a part-time medical assistant.

From well before day one, we immersed ourselves in the Direct Primary Care movement—touring practices across the country, attending conferences, and connecting with other physicians to learn how to build a patient-centered practice. Along the way, I became active in shaping healthcare policy in Oregon. I helped introduce House Bill 2540, allowing patients to apply cash-priced services toward insurance deductibles, saving time and money. I also contributed to Senate Bill 951, one of the nation’s strictest corporate practice of medicine laws, which removes non-competes, gag clauses, and restrictive employment contracts—helping keep physicians independent and local.

Today, our practice has grown to include my wife, Cassandra Elizondo, FNP, and Dr. Autumn Conde, DO, alongside multiple medical assistants and a receptionist, all dedicated to giving patients the time, attention, and care they deserve. In just two years, we’ve not only built a thriving clinic but helped strengthen the DPC community in our area. Our mission remains clear: to make primary care accessible, affordable, and truly patient-centered, while advocating for patients and physicians alike.

Nicholas Jones, MD

I’m a board-certified Family Medicine physician who trained in San Antonio (Go Spurs Go!) and Corpus Christi, where I cared for uninsured and underinsured patients. I grew up in McAllen, Texas, where my family taught me hard work, respect, and helping others. With a family full of teachers, I believe the best way to understand something is to teach it, which is how I care for my patients.

 

I’m passionate about lifestyle and preventive medicine. Direct Primary Care lets me spend time educating and supporting patients, answering questions, and helping each person feel confident and informed about their health.​

Autumn Conde, DO

I’ve been practicing medicine for over 18 years, with experience in emergency and urgent care as well as humanitarian work locally and abroad. I trained at Ohio University Heritage College of Osteopathic Medicine and take a holistic approach to care, blending my osteopathic training with each patient’s unique story.

I’m especially interested in preventative care, integrative medicine, complex cases, and geriatric health. I love teaching and empowering patients to advocate for themselves and their families. Outside of medicine, you’ll often find me reading, swimming, or enjoying the snow—whether skiing, snowboarding, or hiking.

Cassandra Elizondo FNP

Hi, I’m Cassandra Elizondo, a Family Nurse Practitioner. My husband Nick and I moved here from Texas in 2020, and I’ve loved caring for families in this community ever since. Before moving, I worked in pediatrics and spent six years helping kids and families at San Antonio Children’s Hospital as a nurse.

​

What I love most is helping people with everyday health concerns—things like nutrition, mental health, skin issues, diabetes, and high blood pressure. Taking the time to listen to your story and finding practical ways to support your health is why I became a nurse practitioner—and why I love what I do.

Shalynn Guzman

Medical assistant and proud mom with a strong passion for caring for others. In my free time, I enjoy spending time outdoors with my dogs, crafting, gardening, and continuing to learn and grow.

Sydney Staples

I am currently pursing a career in Nursing, with the hope to go into either Pediatrics or the NICU. I love traveling and enjoy being outdoors whether skiing, hiking, or swimming, as well as reading or cooking/baking. I am a Colorado native, but love living here in Oregon!

Linda Sanchez-Vasquez

I am a Medical Assistant and come from a large family, which has greatly contributed to my compassionate and empathetic nature. I am always eager to help others. In my free time, I enjoy hiking and attending concerts. I was born and raised here in Oregon.
